Dermal Filler Aftercare, Done Properly

What to do tonight, what to expect this week, and when to pick up the phone.

Good aftercare protects both your result and your safety. At Dr Harry Clinic, a medical aesthetic clinic in Chiswick and Covent Garden, London, every dermal filler appointment ends with the same conversation: what to do in the first 48 hours, what is normal in the days after, and when to get in touch. This guide, from GMC-registered aesthetic doctor Dr Harry James, covers all of it.

Quick Answer: for the first 24 to 48 hours after dermal fillers, avoid touching or massaging the treated area, strenuous exercise, alcohol and extreme heat. Some swelling, tenderness or bruising is normal and usually settles within days. Your final result shows at two to four weeks once the filler fully integrates.

The first 24 to 48 hours

The first two days are when the treated area is most delicate. Keeping things simple gives the filler the best chance to settle exactly where it was placed.

  • Leave the area alone. No rubbing, massaging or pressing unless Dr Harry has specifically advised it.
  • Skip makeup over injection points for 24 hours to keep the tiny entry points clean.
  • Avoid strenuous exercise for 24 to 48 hours. Raised blood flow can worsen swelling and bruising.
  • Avoid alcohol for 24 hours and stay well hydrated instead.
  • Stay away from extreme heat such as saunas, steam rooms and sunbeds for the first week or two.
  • Sleep slightly elevated for the first night or two and try not to sleep face down on the treated area.

A cool compress helps. A clean, cool (not frozen) compress applied gently for short periods can ease swelling in the first day. Wrap it in cloth and avoid pressing on the treated area.

Your aftercare timeline at a glance

TimeframeWhat is normalWhat to do
First 24 hoursSwelling, redness, tenderness at injection pointsNo makeup on the area, no alcohol, no exercise, gentle cool compress
Days 2 to 3Swelling peaks then starts to ease; bruising may appearResume light activity, keep skincare simple, stay hydrated
Week 1Most swelling and bruising settles; area can feel slightly firmAvoid saunas and intense heat, protect skin with SPF
Weeks 2 to 4Filler integrates and softens into its final lookAttend your review if booked, judge the result now, not on day one

Lips deserve a special mention: they tend to swell more than any other area, often most on the morning after treatment. That is expected, and it is not your final result.

What is normal, and what is not

Normal: swelling, tenderness, light bruising, brief pinpoint bleeding on the day, and small soft bumps that smooth out as the filler settles over the first couple of weeks.

Not normal: severe or increasing pain, skin that turns white, dusky or mottled, blistering, or any change to your vision. These can signal a rare vascular complication that needs immediate attention. Contact the clinic straight away, at any hour, if you notice them. Dr Harry James is a faculty member of ACE Group World, the UK body focused on managing aesthetic complications, and every patient leaves with clear emergency contact instructions.

Longer term: helping your results last

Once the filler has settled, daily SPF, a consistent skincare routine and sensible sun habits all help the result last. How long that is depends on the product and the area treated, covered in our guide to how long dermal fillers last. Booking a review around two weeks after treatment lets us fine-tune anything while the result is fresh. Our complete before and after care guide covers preparation for your next visit too.

Frequently asked questions

When can I wear makeup after fillers?

Keep makeup off the injection points for 24 hours to protect the tiny entry sites. After that, normal makeup is fine, applied gently.

When can I exercise again?

Wait 24 to 48 hours before strenuous exercise. Raised heart rate and blood flow in the first days can increase swelling and the chance of bruising. Gentle walking is fine straight away.

Can I drink alcohol after dermal fillers?

Avoid alcohol for the first 24 hours. It widens blood vessels, which can make swelling and bruising worse. Water is your friend on treatment day.

When will I see my final result?

Allow two to four weeks. Early swelling exaggerates the look on day one, then the filler gradually integrates with your tissue and softens into its true shape.

Can I sleep on my side after treatment?

For the first night or two, try to sleep on your back with your head slightly elevated, and avoid pressing the treated area into the pillow. After that, sleep however you are comfortable.
